Preparing for Gastric Sleeve Surgery: What to Expect and How to Get Ready

Choosing gastric sleeve surgery is a big decision — and a powerful step toward improving your health, wellbeing, and quality of life. Whether your goals are weight loss, mobility, fertility, or simply feeling better in your body, sleeve gastrectomy can be a life-changing tool.

But like all surgery, success starts with preparation. At Cosmedicare, we’ll guide you through every step — but here’s what you can do now to get ready, both physically and mentally, for your surgery and recovery.

Start With the Right Mindset

Gastric sleeve isn’t a “quick fix” — it’s a medical tool that works with you, not for you. The best outcomes come when patients feel informed, empowered, and supported.

  • Ask questions, take notes, and take your time
  • Be honest with your clinical team — we’re here to help, not judge
  • Remember: you’re not expected to be perfect, just committed

2–3 Weeks Before Surgery: Begin the Liver Shrinkage Diet

Most patients will be asked to follow a special liver-shrinking diet (LSD) for 2 weeks before surgery.

Why? The liver sits over your stomach — and when it’s too large or fatty, it can get in the way during surgery. The diet reduces fat stored in the liver, making the operation safer and smoother.

This diet usually includes:

• High-protein meal replacements or shakes

• Low-carb, low-fat vegetables

• No alcohol, sugar, or snacking

• Plenty of water

It’s not easy — but it’s temporary, and crucial for your safety.

Stop Smoking

Smoking significantly increases your risk of complications, poor healing, infections and even blood clots. You must stop smoking at least 4–6 weeks before surgery. If you don’t, your surgeon may delay or cancel your procedure.

Medications and Supplements

At your pre-op assessment, we’ll review all your medications. You may be asked to stop taking:

  • Certain blood thinners
  • Anti-inflammatory drugs (like ibuprofen)
  • Herbal or vitamin supplements

Only stop medication after speaking to our team — never adjust prescriptions on your own.

Get Your Recovery Space Ready

  • Stock up on protein shakes, broth, sugar-free drinks, and soft foods
  • Prepare a calm space to rest, with cushions, blankets, and books or entertainment
  • Arrange time off work (usually 2–3 weeks minimum)
  • Set up support from family or friends for the first few days

Surgery Day: What to Expect

  • You’ll arrive early for admission, where our nursing team will settle you in
  • The procedure takes around 60–90 minutes, usually under general anaesthetic
  • Most patients stay 1–2 nights in our private hospital to rest and recover
  • You’ll be closely monitored and supported throughout

Post-Surgery Diet: Staged and Structured

Recovery includes a carefully structured diet to allow your new stomach to heal:

  1. Liquid phase (first 1–2 weeks): clear liquids, broths, protein drinks
  2. Pureed phase (2–4 weeks): smooth, blended foods
  3. Soft foods (weeks 4–6): soft proteins and gentle solids
  4. Normal textures (from 6 weeks onwards): gradual reintroduction with guidance

You’ll be supported by our dietitian and clinical team every step of the way.

Top Tips for a Smooth Journey

  • Follow your team’s advice — not what others are doing online
  • Hydration is key — sip water regularly to avoid dehydration
  • Prioritise protein — it helps healing and maintains muscle
  • Go slow — recovery is gradual and looks different for everyone
  • Attend all follow-ups — they’re essential to your safety and success

At Cosmedicare and St. Ellen’s Hospitals, your gastric sleeve journey is never rushed or one-size-fits-all. You’ll receive full pre-op assessment with our specialist team, access to dietitians, psychologists, and clinical support. Gastric sleeve surgery is a tool — but you are the driver. The preparation you do now sets the tone for everything that follows. Trust the process, stay consistent and be kind to yourself.
